Technical Assistance:

1. Be sure there is indication on the service plan that the individual was offered a copy of the plan and either accepted or refused. 2. For those persons diagnoses with some form of dementia their plan should reflect their inability to use the call bell system if applicable. In that case "rounds" would have to be described such as every hour or whatever is decided.

Comments:

This inspection was conducted by licensing staff using an alternative remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 9/10/2020 and concluded on 9/16/2020. The administrator was contacted by email to initiate the inspection. The administrator reported that the census was 14. The inspector emailed the administrator a list of items required to complete the inspection. The number of records to be reviewed was based on the census provided. The inspector reviewed two staff records including related training records, two resident records plus medication records from three additional residents, fire inspection, fire drills, menus, activities, pharmacy review, and health care oversight. Background checks for individuals hired since the last inspection were also reviewed. This information was submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law in one area. A violation was documented and reviewed with the administrator and can be found on the violation notice issued to the facility. Description:	Based on a review of staff training records there was not documentation that residents rights were reviewed annually.
Plan of Correction:	Relias training will be reviewed to make resident rights a part of the mandatory staff training. If this cannot be done the administrator will review rights with staff on an annual basis. The administrator assumes responsibility for correction and future compliance.
