1. Discussed water lead testing to be completed by January 2025.
2. Reviewed First Aid/CPR requirements.
3. The violation notice from each inspection should be posted in the facility once received.
4. Environmental health inspections information was sent in February 2024, as a new requirement.
- Comments:

Conducted an unannounced monitoring inspection 12:30-2:10pm. Observed 19 children + 6 direct-care staff. Ratios and supervision were in compliance. Children were observed playing with blocks and toys, having active play in the social hall, having story time and preparing to take naps. Staff were actively engaged with the children and activities. The program areas were found to be clean and sufficiently supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Standard #: 8VAC20-780-160-C Description: Based on records reviewed, 1 of 4 staff records did not include a TB screening every 2 years. Evidence: The most recent TB test for Staff #4 was dated as completed 6/3/22. Plan of Correction: TB test done AM 7/17 to be read 7-18 [sic]
Standard #: 8VAC20-780-260-A Description: Based on interview conducted, the annual fire inspection has not been completed for the facility. Evidence: The program director reported that she requested the fire inspection to facility management staff, but to-date it has not been conducted. Plan of Correction: Fire inspection completed 7/12/24
Standard #: 8VAC20-780-260-B Description: Based on interview conducted, the annual environmental health inspection has
