During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced inspection was initiated on 10/22/2024 and completed on 10/23/2024 in response to a complaint received by the licensing office on 10/11/2024 relating to allegations of supervision and injury reporting. The inspector reviewed two children?s records and one staff record electronically on 10/23/2024. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. Though not a part of the original complaint, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.

Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/07/2024. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-140-A
Complaint related: No
Description: Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance.

Child #1, who has been enrolled for over five years, had no physical on record.

Plan of Correction: Moving forward management will make sure to obtain a physical on each child upon enrolling.

Description: The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures that are required by these standards.

The center's transportation policy on arrival to the child care center with an empty bus states; ?The driver waits on the empty bus for a member of the management team to come outside and check the bus. The driver should never get off the bus until a member of the management team has checked the bus to ensure that no children remain on it. The member of management signs the bus roster indicating that they have completed the 1st child passenger check. The driver walks front to back and back to front to ensure that no children remain on the bus". On 04/17/2024, staff #1 returned to the center from a school drop and parked the center?s bus. Staff #1 exited the vehicle and was unaware that child #1 was in the vehicle asleep.

Plan of Correction: Moving forward if a member of management does not check the bus, the driver must enter the building and find a manager to check the bus including staying in a room while the manager checks bus. From page 2 the manager then signs the bus roster indicating no children are left on the bus.

Description: (Repeat Violation)
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all required information.

Child #2, who has been enrolled for over one year, had no documentation of viewing proof of the child's identity and age.

Plan of Correction: Upon enrolling the director will ensure all required information and documentation is on file. We have asked parents for this document.

Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 04/17/2024, staff #1 returned to the center from a school drop and parked the center?s bus. Center staff were not aware that child #1 was left in the vehicle until approximately 10 minutes later when child #1 (age 6 years) exited the vehicle and walked to the center?s door.

Plan of Correction: Moving forward children will be in sight and sound of all children to ensure care, protection and guidance.

Description: Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On 04/17/2024, Staff #1 transported children to school around 7:30am. Child #1 (age 6 years) fell asleep on the bus during the drive to school. Staff #1 dropped off children at school and returned to the center around 7:45am. Child #1 exited the bus at the center unsupervised and walked to the door 10 minutes after the bus arrived at the center.

Plan of Correction: As of 4-18-2024. The driver will walk to the back of the bus to insure no children are left on the bus.

Description: Staff shall verify that all children have been removed from the vehicle at the conclusion of any trip.

On 4/17/2024, child #1 (age 6 years) was left unsupervised in the center?s bus for approximately 10 minutes by staff #1, who failed to follow procedures to verify that all children had exited the vehicle after arriving back to the center from dropping off children at school.

Plan of Correction: As of 04/18/2024. The driver will walk to the back of the bus and will ask an administrator to come and do another check to insure no kids are on the bus.
