During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
- Technical Assistance:

Effective January 1, 2025, the VDOE will begin determining compliance with 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. An unannounced, on-site monitoring inspection was initiated on 10/29/2024 and completed on 10/29/2024. The on-site inspection began at 10:50am and ended at 12:10pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 1 caregiver; the point total was 13. The inspector reviewed 6 children?s records and 1 caregiver record on-site. This inspection included document review, tour of the home, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/13/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-800-100-A Description: The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
There were no physical exam records available for child #1 (enrolled for over 3 months) and child #2 (enrolled for 7 months).Plan of Correction: The physicals are now in child's folder.

There was no record of attendance for child #4, who was in care on the date of the inspection.Plan of Correction: Parents were told to sign children in and out everyday.
Standard #: 8VAC20-800-60-B Description: The provider shall maintain a separate record for each child enrolled that contains all of the required information.
The record for child #2, enrolled for seven months, was missing the following required components: the name, address, and phone number of 1 designated person to contact in case of emergency. The record for child #3, enrolled for over 1 year, was missing the following required components: phone number and address for 1 parent, employment information for 1 parent, and work phone number for both parents.Plan of Correction: Done
Standard #: 8VAC20-800-180-A Description: A tuberculosis (TB) screening is required for each caregiver and adult household member every two years from the last screening.
An updated TB screening for Caregiver #1 was completed 4 months after being due.Plan of Correction: Will do every two years.
