Comments:
An unannounced focused monitoring inspection was conducted with the director on May 9, 2024, beginning at 10:45 a.m. and ending at 12:05 p.m. Three staff records were reviewed during the inspection. An inspection of the physical plant and observations were completed. Additional facility documentation was reviewed in compliance.

Description: REPEAT VIOLATION: Based on a review of documentation and staff interview, it was determined the center failed to have at least one staff member who meets the qualifications of a program leader or program director shall be regularly present for each grouping of children. Evidence: The teacher in the infant room does not possess program leader qualifications.

Description: Based on observation, the center did not ensure that all mattress being used were covered with waterproof material which can be cleaned and sanitized. Evidence: One crib mattress in the infant classroom had scratches and splits making it no longer waterproof. .
