An unannounced inspection was initiated on 11/21/2024 and completed on 12/04/2024 in response to a complaint received by the licensing office on 11/14/2024 relating to allegations of Programs. The inspector reviewed one staff records on-site. This inspection included: document review, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Though not a part of the original complaint, violations that were not reported were found related to Administration and are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-40-E Complaint related: No Description: Centers shall be in compliance with the center?s own policies and procedures that are required by the standards.
The center has a policy that forbids the rough handling of children. On multiple occasions on 10/17/2024 and 11/12/2024, Staff 1 roughly handled 5 different children age two years old.Plan of Correction: The Director stated that all staff members will have refresher training in this policy, and what to do if staff are frustrated and need a break.

10/17/2024, Staff 1 grabbed Child 1 (age 2 years) by the back of the neck when the child did not clean up their mess. Staff 1?s hand remained holding Child 1 by the back of the neck while the child screamed and cried as Staff 1 made Child 1 clean up the mess.
10/17/2024, Staff 1 physically pushed Child 1 (age 2 years) away from another child, which made Child 1 fall to the ground.
10/17/2024, Staff 1 grabbed Child 2 (age 2 years) by the back of the arm, turned the child towards her, and grabbed the child by the face. Staff 1 told Child 2 to clean up their mess, then pushed the child to the floor, and pushed the child to a cubby to make the child put their toys away. Child 2 was screaming and crying.
11/12/2024, Staff 1 asked Child 3 (age 2 years) to get down off a piece of furniture. When the child said no, Staff 1 grabbed the child?s ankles which caused the back of the child?s head to hit a piece of furniture.
11/12/2024, Staff 1 roughly handled Child 4 (age 2 years) when the child was having a tantrum and didn?t calm down.
11/12/2024, Staff 1 roughly handled Child 5 (age 2 years), carrying the child upside down, when the child would not walk back to the classroom. The child was screaming, kicking, and crying.Plan of Correction: The staff member was suspended as soon as the Director discovered these allegations and is no longer employed at the facility.
