An unannounced mandated inspection was conducted on 4/20/22. At the time of entrance fourteen participants were in care. The sample size consisted of four participant records and four staff records. Resident and staff records and other documentation reviewed. Criminal Background Checks of staff hired since previous inspection were reviewed. Participants were observed playing chair hockey, games, and crafts . Medications were reviewed. No violations given today, and exit interview held.
