An announced file review inspection was conducted today from approximately 10:00am-2:00pm. Zero new staff have been hired since the last inspection. Based on record review, staff records were missing required information.
Evidence: The record of Staff A was missing documentation to verify qualifications for their position and documentation of orientation.
