Comments:
An unannounced Code Compliance inspection was conducted with the director on September 30, 2022 beginning at 11:39 AM and ending at 2:42 PM. There were 76 children, ages 6 months through 5 years old, in care during the inspection. The children were observed playing with age-appropriate toys and doing instructional work. Five children's records were in compliance with the code requirements. Five staff records were reviewed. There were no medications on site to be reviewed. Facility policies and procedures were reviewed and in compliance.

Based on a review of five staff records, the center failed to obtain fingerprint background checks as required. Evidence: Staff A, staff B, staff C, and staff D did not have documentation of completed fingerprint background checks in their records.
Plan of Correction:
The facility will ensure that staff member have fingerprinting completed and documentation placed in their records.

Description:
Based on a review of five staff records, the center failed to ensure that a sworn statement prior to the first day of employment and a central registry background check was obtained within 30 days of employment. Evidence: Staff A (start date 9/5/2022) was missing documentation of a sworn statement in their record. Staff B, staff C, and staff D were missing documentation of central registry background checks from their records.
Plan of Correction:
The center will ensure that the missing documentation is requested and placed in the staff records.
