Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.
A monitoring inspection was initiated on 11/08/2021 and concluded on 11/10/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 62 children present, ranging in ages from 6 months to 5 years ,with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on documentation review, the center did not ensure that each child shall have a physical examination by or under the direction of a physician within one month after attendance. Evidence: 1) The record for Child #2 did not contain a physical examination. There was a CoVA School Entrance Health Form in the record however, it was not completed nor signed by a physician.

Description:
Based on record review and interview with the director, staff's records did not contain all required information. Evidence: 1) The record for Staff #3 did not contain written information to demonstrate that the staff person possesses the orientation training required by the job position. 2) The center director stated that Staff #3 completed the orientation training but they could not locate the documentation in the record.
Plan of Correction:
We will ask the staff person complete a new orientation training form.
