**Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. Comments:
A monitoring inspection was conducted on June 6, 2024 between the hours of approximately 11:30am and 1:15pm. There were 51 children present, with 10 staff members supervising. Children were observed in group play, completing art activities, washing hands, and preparing for lunch.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Repeat violations were found and are identified within this report.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Violations:
Standard #: 22.1-289.035-B-2
Description: Based on record review, the center did not ensure to obtain a fingerprint-based criminal history check determination letter prior to the first day of employment for one staff person.

Evidence:
1) On the date of inspection, the record for Staff #5 (hire date 09/19/2023) contained a criminal background results dated 09/29/2023, which is ten days after the first date of employment.

Plan of Correction: Going forward, we will ensure that the fingerprint-based criminal history check is in receipt before staff begin employment. Staff #5 had to re-do the FBI check due to a typo discovered later.

Standard #: 8VAC20-770-60-C-2
Description: Based on record review, it was determined that the center did not ensure that Central Registry search results were obtained by the end of the 30th day of employment.

Evidence:
1) On the date of inspection, the record for Staff # 3 (hire date 05/28/2024) and staff #5 (hire date 09/19/2023) did not have a current central registry background check on file.

Plan of Correction: We contacted the Central Registry office. They will email us a copy of #3 within 10 business days, and a copy of #5 in 10 business days.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances were not kept in a locked place.

Evidence: On the date of inspection it was found that bleach sanitizer spray, multi-surface cleaner, and disinfectant spray, were kept in an unlocked cabinet in the Preschool classroom (directly over the changing table) and/or making them accessible to children.

Plan of Correction: A new lock was secured on this cabinet in the Preschool Classroom.

Standard #: 8VAC20-780-550-C
Description: Based on observation and interview, the center did not ensure that emergency evacuation and shelter-in-place procedures or maps are posted in a location conspicuous to staff and children on each floor of each building.
Evidence:
1. The center was toured and there was no evidence of an emergency evacuation and shelter in place map posted in the main lobby, preschool, toddler, and infant classrooms.
2. The site director confirmed the emergency evacuation map was not posted due to the building being recently painted.

Plan of Correction: Each classroom received a new map displaying emergency evacuation and shelter-in-place procedures. Also posted on Parent Board.
