Comments:
An unannounced renewal inspection was conducted on 8/28/19 from 12:20pm until 3:30pm. At the time of the tour, there were 81 children in care with 11 staff present. A sample of 8 children's records and 8 staff records were reviewed. Children were observed resting quietly during nap time and playing outside on the playground. Restroom and hand washing procedures were also observed. Medication, transportation, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Five violations in the areas of administration and physical plant are identified on the violation notice and were discussed with the center director during the exit interview.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review and interview, the center did not ensure that documentation that each child has received the immunizations required by the State Board of Health was obtained before the child can attend the center.

Evidence:
1. Child 1 has a date of enrollment of 2/12/18 and documentation of immunizations was not obtained by the center until 8/17/18.
2. The center director confirmed that documentation of immunizations for child 1 were not obtained before the child's first day in care.

Plan of Correction: Going forward, the center will ensure documentation of current immunizations are obtained before a child can attend the center.

Standard #: 22VAC40-185-130-B
Description: Based on record review and interview, the center did not ensure that documentation of additional immunizations are obtained once every six months for children under the age of two years.

Evidence:
1. 1. The most recent immunization record on file at the center for child 1, age 1 year, is dated 8/17/18. Child 1 was due for an updated immunization record in February 2019 and August 2019.
2. The center director confirmed that updated immunization records for child 1 were not obtained within the required time frame.

Plan of Correction: The center contacted child 1's parent during the inspection and requested an updated immunization record.

Standard #: 22VAC40-185-140-A
Description: Based on record review and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Evidence:
1. The record for child 1 does not contain documentation of a physical examination. Child 1 has a date of enrollment of 1/12/18 and was in care on the day of the inspection.
2. The center director confirmed that the record for child 1 is lacking documentation of a physical examination.

Plan of Correction: The center contacted child 1's parent during the inspection and requested a copy of the most recent physical examination. Going forward, the center will ensure that documentation of physical examinations for each child are obtained within the required time frame.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure that areas of the center shall be maintained in a clean and safe condition.

Evidence:
1. There are multiple areas of peeling paint along the back wall of the school-age classroom.
2. The center director confirmed that there are areas of peeling paint in the school-age classroom.

Plan of Correction: A work order will be placed today to have the areas repainted.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances, such as insecticides, shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In the school-age classroom, there was a can of wasp and hornet spray on the top shelf of an unlocked closet and was accessible to children in care. The container stated "caution", "keep out of reach of children", and "avoid contact with skin, eyes and clothing."

Plan of Correction: The can of wasp and hornet spray was removed from the closet during the inspection and placed in a locked area not accessible to children in care. A work order will be placed for the lock to be replaced on the closet in the school-age classroom.
