Comments:
An unannounced monitoring inspection was conducted with the center staff to determine compliance with licensing standards. There were 50 school age children present with 8 staff actively supervising. The children were arriving from school via public school bus and center vans. The children engaged in free play activities indoors; the heat index prevented outdoor play today. Snack served today: popcorn and juice. Nine children's files were reviewed - please see Supplemental Summary for details. There are no medications on site today; five staff are certified in Medication Administration Training (MAT). Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of seven children's files, the facility failed to have documentation of viewing proof of the child's identity and age within seven days of enrollment for each child. Evidence: Child 3 (date of enrollment 8/19/2019) and Child 9 did not have documentation of proof of age and identity on file.

Plan of Correction: Child 3 now attends public school (starting 9/3/2019) and the center assumes responsibility for the child directly from the school. Center staff requested a copy of Child 9's proof of identity for the file on 10/2/2019.

Standard #: 22VAC40-185-230-B
Description: Based on staff interview and file review, the facility did not keep an independent contractor within sight and sound supervision of a staff member. Evidence: Child 9 was alone with an independent tutor and was not in sight and sound supervision of a staff person.

Plan of Correction: Center staff will ask parent to sign permission for the tutor to sign the child in and out or will have a staff member supervise the independent tutor while on site.
