The licensing inspector conducted an unannounced inspection on 04/10/2024 In response to a self--reported incident regarding allegations in the area of behavioral guidance and forbidden actions. Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a self-report by the center, interviews with staff and a record review the center did not ensure physical punishment such as striking a child and rough handling did not occur.
Evidence: 1. The center self- report that on 04/08/2024 Staff #1 yanked Child B by his jacket pulling the child back towards her, causing him to fall to the ground. It was also reported by the center that on the same day, Staff #1 spanked Child A for not listening to her.
2. A parent and a staff member reported witnessing Staff #1 roughly yank Child B by the front of his jacket causing him to fall to the ground while on the playground.
3. Staff #2 reported witnessing Staff #1 on another occasion using her hand to swat the front of Child A's chest when he was standing on a chair.
4. Staff #1 had been previously written-up by management for ?swatting a child," on 01/29/2024. The document was reviewed in the staff's file and staff #4 confirmed the incident.
5. Staff #1 was described by her co-teacher as "aggressive" during an interview.
Plan of Correction:
Staff #1 was immediately placed on administrative leave. Once the investigation is complete, the staff member will be terminated.

Evidence: 1. Staff #3 and #4 confirmed Staff #1 told Child B she was going to "two piece him up" if he did not listen.
2. Two staff witnesses confirmed that Staff #1 "cusses" at the children. Staff reported she often tells the children to "go sit your *ss down."
Plan of Correction:
Staff #1 was immediately placed on administrative leave. Once the investigation is complete, the staff member will be terminated.
