A subsidy inspection was conducted on 07/14/2022. Staff and children's files were reviewed, interviews conducted with staff in addition to a tour of the facility. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a record review the vendor did not ensure all staff had subsequent tuberculosis screenings at least every 2 years since the last screening. Evidence: Staff A's last TB screening was dated 08/16/2018.
Plan of Correction:
The staff had a chest x-ray done and thought it was good for years, per physicians instructions. A TB screening will be obtained this week.

Based on a record review the vendor did not ensure an annual an annual certification from the Health Department. Evidence: The health last inspection provided by the vendor was dated 03/04/2020.

The vendor will contact the health department this week and schedule an inspection.

Description:
Based on a record review the vendor did not ensure emergency drills were practiced at least monthly. Evidence: The vendor did not have documentation of performing evacuation or lock down drills.
Plan of Correction:
The vendor has performed emergency drills, however due to a transition is director, the documentation has been misplaced. The vendor will ensure all emergency drills are performed and documented.
