Comments:
An unannounced monitoring inspection was conducted from 9:20 am-12:00pm. At the time of entrance, 77 children were in care with 14 staff members present. Children were observed during outside play time. Infants were observed being fed, napping and playing with toys. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications and the Emergency Preparedness Plan were reviewed. . Based on review of seven children's records, the center did not obtain updated immunization records every 6 months for children under the age of 2. Evidence: 1. Child #5 had an immunization record dated 8/29/2018 and needed an updated record in the file.

The parent was contacted during the inspection and a copy was requested.

Description:
Based on review of seven children's records, the center did not obtain documentation of a physical in each child's file prior to or within 30 days of attendance. Evidence: 1. Child #5 began at the center on 8/06/2018 and the center did not have a copy of the child's physical.

Description:
Based on observation, areas and equipment of the center inside were not maintained in a safe and operable condition. Evidence: 1. In the Pre-K classroom, the paint was peeling under the 2nd window sill to the left. 2. In the School Age classroom, the paint was peeling to the right of the sink near the door. 3. In the School Age classroom, the sofa is visibly stained and dirty. 4. In the Discovery preschool classroom, the carpet edge is fraying and coming apart.
Plan of Correction:
A new rug was ordered during the inspection. A work order was sent in for the peeling paint. The sofa will be covered with a new sofa cover.
