Comments:
An unannounced renewal inspection was initiated and concluded on July 26, 2022. There were 55 children in care ages two through 10 years old with five staff and one administrator. Children and staff records were reviewed. A walking tour of the facility inside and the outside play areas. Children were observed in teacher and self-directed activities. Violations regarding accessibility of children's record information was found and discussed with the program director at the conclusion of the inspection.
Based on record review and interview the center failed to ensure each child's record contains all required information and documentation. Evidence: Five out of five children's records reviewed did not contain documentation of first date of attendance or required health information.
Plan of Correction:
The information requested was at the center but not accessible during the inspection. Going forward center management will ensure that all required children records information is available for review during all hours of center operation.
