Comments:
A monitoring inspection was conducted on 11/02/2022. T A tour of the facility was conducted, in addition to a record review and interviews with staff. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-330-B
Description: Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence:
There were cracks in the poured-in-place resilient surfacing in the fall zones of both slides on composite play structure.

Plan of Correction: A work order has been placed for awhile. There was turnover in the maintenance department causing delays in work orders. The director will contact the DM in regards to having the surfacing repaired.

Standard #: 8VAC20-780-340-F
Description: Based on observation the center did not ensure children under 10 years of age were always within actual sight and sound supervision of staff.
Evidence:
In the small foyer area in front of the toddler room, there was a 1 year-old child observed napping with no supervision.

Plan of Correction: Normally the half door leading to the foyer area in front of the toddler room is left open. The door had been accidently closed while staff was changing diapers. The child was napping there per request of the parent. The staff will have the child nap inside the toddler room in an area away from the other children from now on.

Description: Based on observation the center did not ensure all requirements were met when sunscreen is used.
Evidence:
There were several bottles of sunscreen on a cart at the exit door to the playground. The sunscreen bottles were in reach of children.

Plan of Correction: The sunscreen was left from the summer and is no longer being used. It will be discarded.
