Comments:
An unannounced monitoring inspection was conducted today from 12:30pm-2:00pm. There were 24 children (2yrs-5yrs.) directly supervised by 10 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed napping. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Based on record review, the center did not maintain a current written list of all children's allergies, sensitivities and dietary restrictions.
Evidence: The written list of all children's allergies, sensitivities and dietary restrictions was dated 8/2021 - 8/2022.

Created a new sheet with updated school year.

Evidence: The records of Child #2, #3 and #4 are missing the mothers' work phone number.

These student's parents do not have work numbers. An "NA" was written in space by parents.

Description:
Based on record review, the center did not have documentation that two or more references as to character and reputation as well as competency were checked for a staff member before employment.
Evidence: Staff #3's record is missing 1 of 2 reference checks required before employment.
Plan of Correction:
Asked employee to turn in a second reference as soon as possible.
