Comments:
An unannounced monitoring inspection was conducted today from 9:15am-10:55pm. There were 40 children (2yrs.-5yrs.) directly supervised by 9 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, coloring, participating in organized activities, reading books, playing outside, and eating breakfast. There was an adequate supply of materials for the children. The center was clean and organized. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, children's records did not all contain required information.
Evidence: 1.Child #1's record does not contain the address for one of the two required emergency contacts. 2.Child #3's record does not contain documentation that the facility verified the child's proof of identity within seven business days of the child's first day of attendance.

Remind parents to fill out all sections. Asked parent to bring in proof to be verified.

Description:
Based on observation, equipment was not maintained in a safe and operable condition.
Evidence: In the Cubs classroom, a plastic mat in the reading nook/library area was observed with several tears, approximately 10 inches in length, all over the top area of the mat.

Remove mat and replace with a rug.

Description:
Based on review of emergency procedures, emergency drills were not practiced monthly.
Evidence: An evacuation drill was not practiced for the month of April 2023.

Remind classrooms of assigned fire drills and set a week for it to be done.
