Comments:
The licensing inspector conducted an unannounced monitoring inspection on 12/04/2019 from 10:45am to 2:30pm. The children were observed learning through play in centers, participating in story time, eating lunch and preparing for nap. A formal observation was completed with the Toddler and CC II classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today.
Seven children?s records and nine employee records were reviewed during this inspection. Medication is being administered and was reviewed. Ten injury reports were reviewed as well as the center?s injury prevention plan which was updated 07/05/2019.
The center's first aid kit and emergency supplies were inspected. Last emergency drill: 11/29/2019 Last shelter-in-place drill: 10/25/2019 Last fire inspection: 07/26/2019 Last health inspection: 07/13/2019
Today, the following child to staff ratios were observed: Infants - 5:2 Toddlers ? 10:2 CC I (Twos) - 12:2 CCII (Twos and Threes) - 13:2 CC II (Threes) - 18:2 CC III(Threes and Fours) ? 15:2
The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection.

Based on observation, the center did not ensure that areas and equipment of the center were maintained in a clean, safe and operable condition. Evidence: The Toddler, Twos and Older Twos classroom had areas located on their window sills with chipped and peeling paint.

Description:
Based on observation and record review, the center did not ensure four infant records contained documentation of written parent authorization noting any known adverse reactions when diaper cream was used. Evidence: 1. The record of child #4, child #5, child #6 and child #7 did not contain written parent authorization noting any adverse reactions for diaper cream. 2. Child #4, child #5, child #6 and child #7 had diaper cream labeled with their individual names. 3. The infant teacher stated that all diaper creams were used as needed within the past month.
Plan of Correction:
We will send the new forms home to be signed. We will put a new system in place to ensure we have the correct form with the correct cream.
