Comments:
Today an unannounced Renewal Inspection took place between the hours of 12:35pm-2:25pm. There was a total of 77 children with 15 staff within the supervision guidelines. The children were observed eating their lunch, taking a nap and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log and emergency procedures were observed during this inspection. Violations:
Standard #: 8VAC20-770-60-B
Description: Based on a review of staff records, all staff did not have a completed sworn statement or
affirmation on file prior to employment.

Evidence:
1. Staff #4 (doh: 4/1/2024) had a sworn statement on file that was dated 4/11/2024, which is after the first date of employment.

Plan of Correction: We will make sure staff sign their Sworn statement by day 1 of employment, within the required timeframe moving forward.

Standard #: 8VAC20-780-270-A
Description: Based on observation, Areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.

Evidence:
1. There was peeling white paint on the table next to the water fountain in the Discovery Preschool classroom.
2. The vent in the Preschool classroom has a large air vent that was observed to have a large amount of build-up on the outside of the vent.
3. The rubber carpet strip in the Pre-K classroom was broken and is not longer adhered to the ground.
4. There were small grey plastic blocks observed on the playground that were broken, leaving sharp edges exposed.

Plan of Correction: We will get all of these items corrected within 4 weeks or sooner. The vent will be cleaned, paint will be covered or re-painted, we will secure the carpet strip and remove the broken toys from the playground.
