An unannounced inspection was conducted in response to a self-reported incident received in the licensing office on 10/16/19. Interviews were conducted with staff, records reviewed and observations of the program were made. A violation was cited in relation to the report and are documented in the violation notice.
Based on a self-reported incident, record review, interviews and observations made of the program, the center did not ensure the use of physical punishment was not used as a method of behavioral guidance. Evidence: On 10/11/19 a staff member reported to the director that she witnessed Staff #2 use physical punishment with children as a form of discipline. Two staff members witnessed and confirmed Staff #2 has used her fingers to flick a 3 year-old child #2's hand as a form of discipline. It was also reported that Staff #2 took the belt from her waist and strapped a 2 year-old child to a chair to restrain him while eating. The program director confirmed staff #2 admitted to both forms of physical punishment during an interview.
Plan of Correction:
The following corrective action was provided by the program director during the exit interview: Staff #2 was terminated from the position. Training on positive discipline methods and mandated reporting will be reviewed with all staff at the next staff meeting.
