Comments:
An unannounced monitoring visit was done today August 21, 2019 beginning at 6:20 A.M. and ending at 8 AM.. There were 24 children in care today with four staff. The children were observed during arrival, table toys breakfast and departure for school.
The inspector reviewed two staff's and five children's records. One medication was reviewed.
Discussion with director about: first aid kit, custody paper work and playground.
Based on review of two staff's records, the center failed to ensure that all TB test/screenings were complete every two years.
Evidence:
1. Staff # 1 most recent TB test/screening was completed on 4/17/2017.
Plan of Correction:
The site director has informed the staff person that a TB screening/test is due. All dates will be monitored.
