Comments:
An unannounced renewal inspection was done today 8/22/2019. The inspector arrived at 6:30 A.M. and departed at 8 am. There were 35 children present today with four staff supervising. The inspector reviewed five children's and three staff's records during the inspection.
Observed the children during: arrival, free play and departure into school.
Based on review of five children's records, the center failed to ensure that all immunization records were signed by a physician or physician designee.
Evidence:
1. Children # 3 and 4 did not have signatures from the physician or his designee on the immunization record.
Plan of Correction:
The site director will have the parent to obtain signatures as soon as possible.

Description:
Based on review of three staff's records, the center failed to ensure that all repeat TB screenings/test were completed every two years.
Evidence:
1. Staff # 3 most recent TB was 7/19/2017.
Plan of Correction:
The site director will have the staff person to obtain a screening or test ASAP.
Standard #:
22VAC40-185-540-C
Description:
Based on review of the first aid kit, the center failed to ensure that all items were included.
Evidence:
1. The first aid kit was missing one of the two required triangular bandages.
Plan of Correction:
The site director will obtain the missing component for the first aid kit and will monitor.
