Comments:
On August 26, 2019 the department received a complaint regarding inappropriate staff interactions with children and supervision of children. The licensing inspector maintained communication with the an investigating outside agency during the investigation. The licensing inspector conducted an inspection at the facility on 10/28/2019 from approximately 10:45 AM until 12:50 PM. The investigation continued off site through a review of documentation on 10/28/19 upon receipt of documentation from an outside agency. On 11/20/19 the licensing inspector returned to the facility and conducted an additional inspection from approximately 10:45 AM to 4:15 PM. Interviews with staff were completed, observations were made and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Based on review of three caregiver records, two records did not contain documentation of a national fingerprint-based criminal history record check within the required timeframes. Evidence: 1. The licensing inspector observed that the record for Caregiver #1 (hire date 10/9/17) contained a national fingerprint-based criminal history record check dated 1/25/19. For caregivers employed prior to 1/22/2018, a statement of eligibility based on a national fingerprint-based criminal history record check was required to be obtained prior to 9/30/2018. 2. The record for Caregiver #2 (hire date 10/21/19) contained a national fingerprint-based criminal history record check dated 11/13/19. A national fingerprint-based criminal history record check is required to be obtained prior to employment.
Plan of Correction:
Administration will ensure that fingerprinting is completed within the required timeframes.
