Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted September 24, 2019. The on-site inspection was held from 9:50 a.m. to 1:35 p.m. On this date, there were 107 children in the care of 15 staff members. Two additional staff members were available to assist, if needed. The total enrollment for the program was 157 children. There were ten staff members' records reviewed, ten children's records reviewed, and the medication and medication paperwork were reviewed for three children. Six classrooms were toured and four of those classrooms were observed. Nine staff members and five children were interviewed. An exit interview was conducted on this date. Some items observed and reviewed included: injury reports, evacuation and shelter-in-place drills, required posted information, warm water temperature, Montessori work activities, math activities with an abacus, music and art activities, Pre-K center rotations, outside play, work with the Letter E,e, story time, transitions, hand washing and bathroom procedures, lunch, and nap time.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and conversation with staff, there was a hole in the drywall in one classroom which created a possible hazard to the children.

Evidence:
1. In Classroom A, there was a hole, a couple of inches in diameter, in the drywall in a wall toward the back of the classroom near the diaper changing area. The hole was close to the floor and was accessible to the children in care.
2. Staff Member A stated that they had just removed a gate which left the hole.

Plan of Correction: The center will have a maintenance person fix the hole as soon as possible.

Standard #: 22VAC40-185-500-B
Description: Based on observation and verification from staff, the center failed to keep a non-absorbent changing pad on the diaper changing table in one classroom.

Evidence:
1. In Classroom A, the vinyl-covered changing pad on the changing table contained holes and tears, making the surface of the pad absorbent.
2. Staff Member B reported that she was aware the changing pad was worn and was ripped.

Plan of Correction: The staff ordered a new changing pad during the inspection. In the future, the center will change out the diaper changing pads as soon as they become non-absorbent.
