Based on review of records, the center failed to obtain two out of state Abuse and Neglect Searches for staff by end of the 30th day of employment. Evidence: 1. Staff #3?s file indicated hire date of 06/13/2022. 2. Staff #3?s Sworn Statement listed Virginia and one additional state as lived in within the last 5 years. 3. A driver?s license provided from Staff #3 was from a third state not listed on the Sworn Statement indicating residency within the last 5 years. 4. Staff #3 confirmed that they had lived in all three states within the last 5 years. 5. Staff #5 confirmed that an Out of State Abuse and Neglect Search had not been completed for Staff #3 from either of the 2 additional states.
Plan of Correction:
Prior to employment, The Virginia State police fingerprint background check came back saying that employee #3 is eligible as an employee. The central registry check came back and stated that employee #3 was not identified in the central registry of child abuse and neglect. On employee #3's sworn disclosure statement the employee listed that he lived in Colorado in the past five years. Prior to employment I checked the Colorado internet criminal history record check and no records were found for employee #3. In our opinion, this was not an error on our part because the employee did not list the other state that the employee had lived in in the past five years.

Lunchboxes were not clearly dated and labeled in a way that identifies the owner in the Classroom #4. 2. Staff #5 and Staff #6 confirmed that lunchboxes in Classroom #4 were brought from home and not clearly dated and labeled in a way that identifies the owner.
