Comments:
An unannounced monitoring inspection was conducted today from 1:30pm-3:50pm. There were 82 children (5-13 years.) supervised by 5 direct care staff. The physical plant, 3 staff records, 8 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed transitioning into the Extended Day program, washing hands, eating snack, and completing homework. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Description: Based on record review, the program did not maintain documentation of a minimum of two shelter-in-place practice drills per year.

Evidence: The Extended Day program did not have documentation of completed shelter-in-place drills for 2018.

Plan of Correction: We will meet the two required drills per year.

Standard #: 22VAC40-185-550-M
Description: Based on record review, 3 of 5 injury reports did not contain documentation of staff and parent signatures or two staff signatures.

Evidence: Three injury reports only contained one staff signature.

Plan of Correction: Corrected during inspection.
