63.2(17) License & Registration Procedures

Technical Assistance:
Be sure the components required for licensing orientation are documented on the first day of employment, even though there is a two day orientation offered at a later date.

Comments:
An unannounced monitoring inspection was conducted with center staff on July 19, 2019 to determine compliance with licensing standards. There were 73 school age children present with nine staff actively supervising. The children were playing in the gym, swimming in the indoor pool, and playing with Legos. Seven children's files were reviewed and are in compliance. Medications for ten children were reviewed; four staff are current in Medication Administration Training. Time of today's inspection: 1:30 p.m. to 4:00 p.m.

Violations:
Standard #: 22VAC40-185-210-A-4-b
Description: Based on review of four staff records, the facility failed to have documentation of 24 hours of lead teacher training for each staff in a lead teacher (counselor) position. Evidence: Staff 2 (date of promotion 12/2018) and Staff 3 (date of promotion 6/2019) did not have documentation of 24 hours of lead teacher training within 6 months prior to promotion or within one month after promotion to lead teacher. Staff 3 has 14 hours of lead teacher training and Staff 3 has 2.5 hours of lead teacher training.

Plan of Correction: Staff will take 24 hours of lead teacher training within 6 months of being promoted to the position or within one month after being promoted to the position.

Standard #: 22VAC40-185-510-N
Description: Based on review of medications and staff interview, the facility did not return medications to parents within 14 days of the expiration of the authorization. Evidence: the short term medication authorizations for Child 5, Child 6, and Child 9 expired after 10 days; the children no longer attend the program. The medications were not returned to the parents within 14 days or discarded or destroyed.

Plan of Correction: The parent will be contacted to pick up the medication or the medication will be discarded.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of four staff records, the facility failed to provide documentation that the required out of state Child Protective Services checks were conducted in states where employees have lived in the past five years. Evidence: Staff 1 and Staff 3 did not have documentation of having sent an out of state CPS check to states where the employee has resided in the preceding five years.

Plan of Correction: Human Resources has a new system in place to ensure the out of state CPS checks are sent and checked on when not received.
