Comments:
An unannounced monitoring inspection was conducted on 3/31/2023 from 9:55 am-11:08 am. There were 12 children (31 points) directly supervised by 3 caregivers. The home, 3 caregiver records, 5 children?s records, 2 household member records, evacuation drills, and policies were inspected. Areas of non-compliance are identified in the Violation Notice. Based on observation, the center did not ensure each first aid kit had all the required items.
Evidence: Two triangular bandages were missing from the first aid kit.

Description:
Based on record review and interview, proof of identity was not obtained within 7 days.
Evidence: 1. The birth certificate number and issue date for child #1 are missing from the proof of identity documentation on file. 2. The date of enrollment for child #1 is 1/17/2022.

I will have the parents bring the original birth certificate.

Description:
Based on observation, potentially poisonous substances and materials were not stored away in an area inaccessible to children.
Evidence: A container of air freshener and disinfectant spray was found under a sink cabinet in the children's bathroom.
Plan of Correction:
Removed during the inspection and I will put the lock back on.
