Based on review of four staff records, the center did not have documentation that two or more references as to character and reputation as well as competency were checked before employment for all staff. Evidence: The references on record for Staff A, with a start date of 8/17/20, were dated 9/17/20.

Description:
Based on review of four staff records, the center did not obtain documentation of the results of a national fingerprint background check prior to date of hire for all staff. Evidence: The records for Staff A (with a start date of 8/17/20), Staff B (with a start date of 8/31/20), and Staff C (with a start date of 8/31/20) did not contain documentation of the results of a national fingerprint background check.
Plan of Correction:
The incorrect fingerprint background check was completed for these staff before they started. There were state fingerprint background checks on file before the staff started. The staff will be scheduled to complete the national fingerprint background checks on 9/25/20, and moving forward we will make sure that the correct checks are done before staff start.
