Technical Assistance:
Discussed Phase II training, records, out of state background checks.
Comments:
An unannounced monitoring inspection was conducted during the after-school program from 2:00pm-5:00pm. There were 31 children (5yrs-14yrs.) directly supervised by 3 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, coloring, working on their homework, eating snack, and playing in the gym. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Based on record review, the center did not obtain a tuberculosis (TB) screening/test within 21 days of employment.
Evidence: Staff#3's record (DOH : 8/27/2019) does not include a TB screening/test.

We will ask the staff member to obtain and submit a TB test screening.

Description:
Based on record review, the center did not obtain the results of a follow-up TB test or screening results for each staff member at least every two years from the date of the last screening or testing.
Evidence: The record of Staff#4 contains a TB test/screening that was last updated on 10/21/2014. The record of Staff#5 contains a TB test/screening that was last updated on 1/25/2008.
Plan of Correction:
We will ask the staff members to bring a current TB screening or test.

Description:
Based on staff record review, there is no documentation that Child Abuse and Neglect checks were initiated for employees that have resided outside the state of Virginia within the last 5 years.
Evidence: Staff#3 (DOH: 08/27/2019) and #4 (DOH: 08/27/2019) have resided outside the state of Virginia within the last 5 years, their records did not have documentation that out-of-state child abuse and neglect results were obtained.

We will send the link to the staff member so they can submit the paperwork.
