Comments:
An unannounced renewal inspection was conducted from 9:15am-12:30pm. At the time of entrance, 53 children were in care with 24 staff members present. Children were observed during hand washing, singing at circle time, learning about planets, counting in Spanish, dancing with streamers, listening to the teacher read a story, sunblock application, toy play with dinosaur, playing in the play gallery inside, engaged in group activities and outdoor play time. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, evacuation drills, attendance records, injury reports, medications and the Emergency Preparedness Plan were reviewed. . Violations:
Standard #: 22VAC40-185-130-B
Description: Based on review of five children's records, the center did not obtain documentation of additional immunizations once every six months for each child under the age of two years. Evidence: 1. Child #1 (start date 11/01/2016) had an immunization record dated 11/01/2016 and did not have documentation of additional immunizations once every six months while the child was under age 2.

Plan of Correction: The family was emailed during the inspection to request an updated immunization record.

Standard #: 22VAC40-185-240-C
Description: Based on review of five staff records, the center did not have documentation that staff received 16 hours of staff development activities annually. Evidence: 1. Staff #5 did not have documentation of 16 hours of training during the school period 9/01/2017 through 8/31/2018. Staff #5 had completed 14 hours.

Plan of Correction: We will make sure a minimum of 16 hours is completed for each staff member.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center inside were not maintained in a safe and operable condition. Evidence: 1. A bench (covered with carpet) in the hallway had carpet fraying off of it and was coming unattached to the bench. 2. Room 128 had peeling pant on the back wall. 3. A carpet mat in the inside play gallery area was fraying in one corner.

Plan of Correction: If the bench can not be repaired, it will be disposed of. I put in a work order to have the room painted. I cut the frayed edge of the carpet.
