Comments:
An unannounced monitoring inspection was completed on July 30, 2019 from approximately 10:00a.m. to 1:30p.m. Two hundred and sixty-two school age children were in care and forty-four staff were on premises. Staff and children were observed at the outdoor camp locations engaged in activities to include swimming, canoeing, fishing, and archery. Interviews and interactions were conducted and medications were reviewed. Ten children?s records and five staff emergency contacts were reviewed. Complete staff records will be reviewed on a different date at the corporate office where they are maintained. The Director, clinic staff and Transportation Coordinator were available for the inspection. The Director was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations:
Standard #: 22VAC40-185-510-E
Description: Based on observation and interview, the licensee failed to ensure that medication is labeled with the dosage and time to be given. Evidence: 1. A prescription epi-pen was observed in a bag labeled with Child #12's name. The epi-pen was not in the original, labeled container and lacked the dosage amount and time to be given. 2. In an interview, Staff #1 reported that center staff found the epi-pen in Child #12's possession when he began the program. Although the center has an authorization, Staff #1 acknowledged the epi-pen is not labeled as required.

Plan of Correction: The Clinic Assistant will contact the parent to ask for correct label.

Standard #: 22VAC40-185-550-M
Description: Based on a review of records, the licensee failed to maintain all required elements of a written record of children's serious and minor injuries in which entries are made the day of occurrence. Evidence: 1. Injury reports from June 17, 2019 to July 15, 2019 were reviewed. Approximately one fourth of the reports did not contain the time the parent was notified. 3. Administrator #1 acknowledged these reports did not contain this information.

Plan of Correction: The Director will have staff consistently add date/time to all incident reports.
