Comments:
An unannounced monitoring inspection was conducted from 12:30 pm to 1:15 pm. At the time of entrance, 5 children totaling 8 points were in care with the provider. The record sample size consisted of 5 children's records and the provider's record and the household member's record. The children in care were observed engaging with the provider, having lunch, and free play. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.
Based on record review it was determined the provider did not ensure that each child's record contained all of the required information. Evidence: The record for child #1 did not contain the complete address for 1 of the 2 required emergency contacts.

Provider will request missing information from parent at pick up.

Description:
Based on observation it was determined the provider did not ensure that all potentially poisonous substances were in areas inaccessible to children. Evidence: There were an open container of Clorox wipes in the bathroom used by the children in care.
