Comments:
A renewal inspection was initiated and concluded on 12/28/2023. There were 7 children present with the provider and 1 household member supervising. The children were observed during free play activities and practicing a shelter in place drill. A total of 5 child records and 2 household member records were reviewed. The inspection started at 9:30 am and concluded at 10:30 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review and staff interview the licensee did not ensure that they shall annually review, with the parent, the emergency contact information required to ensure the information is correct and obtain the parent?s signed acknowledgement of the review.
Evidence: Child #1 (start date 1/2/2019) most recent review was dated 10/25/2021 and Child #2 (start date 9/3/2019) most recent review was dated 9/3/2020.

Provider will have parent's review emergency contact information & sign to acknowledge it is correct.

Description:
Based on record review and staff interview the licensee did not ensure, before the child?s first day of attendance, parents shall be provided in writing all necessary information.
Evidence: Staff #1 confirmed that the following required items were not documented in child #5's record: 1. Policies for the administration of medications. 2. Requirement for the family day home to notify the parent when the child becomes ill and for the parent to arrange to have the child picked up as soon as possible if so requested by the home. 3. Requirement for the parent to inform the family day home within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease. 4. Provisions of the family day homes emergency preparedness and response plan.

Provider will get required documentation for the child's file.
