Comments:
An unannounced monitoring inspection was conducted on 8/16/19 approximately 10:30 am to 1:45 pm. There were 54 children in the direct care of 9 staff members. Children were observed utilizing the gym for free play, group center activity, eating a snack, participating in circle time. Areas of non-compliance are listed on the violation notice. Based on children's record review, 1 of 5 children records did not not contain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: The last documented immunizations record for Child #4 was dated 8/14/18.

I will have the parent get the updated form from the doctor.

22VAC40-185-270-A
Description:
Based on observation, the center did not maintain areas and equipment of the center, inside and outside, in a clean, safe and operable condition.
Evidence: In the classroom of infants, 6 of the 12 cribs were determined not to meet the Consumer Product Safety Commission standard. The requirement is all cribs are manufactured no earlier than June 28, 2011.
Plan of Correction:
I will contact the Director to make sure the cribs are replaced.

Description:
Based on direct observation, the center did not keep hazardous substances in a locked place using a safe locking method that prevents access by children.
Evidence: In the Pre-K classroom, there was a can of shaving cream, that was labeled keep out of reach of children, located in a unlocked cabinet.
Plan of Correction:
I will make sure they are locked and out of the reach of children.

Description:
Based on direct observation, the center did not labeled food brought from home.
Evidence: In the classroom for two-year-old children, food brought from home was labeled with names but not dates.
Plan of Correction:
I will make sure the teachers are provided with tape to label food.

63.2(17)-1720.1-B-3
Description:
Based on record review, 1 of 5 staffing records reviewed did not contain documentation of the results of a search of the central registry maintained by any other state in which the individual has resided in the preceding five years.
Evidence. Staff #3 did not have documentation of an out of state central registry check.
Plan of Correction:
I will work on it this afternoon and have it completed Monday.
