Comments:
An unannounced monitoring inspection was conducted on February 27, 2020 from approximately 9:40 am to 12:00 pm. Two staff were present with twelve children. Five children records were reviewed. Training and emergency contact information were reviewed for the two staff present.
Based on observation, facility did not ensure the areas and equipment of the center are maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.
Evidence: The counter border at the children's sink is separating causing splintered wood and sharp points.
Plan of Correction:
Staff will put in a maintenance request to have it repaired.

Description:
Based on review, facility did not ensure required information is documented when a child sustains an injury.
Evidence: The record for child #2 had an accident/incident log form completed on October 17, 2019 that did not document parent notification (date, time and method), staff present, plan of preventing reoccurrence and two signatures.
Plan of Correction:
Staff will use an updated form when documenting injuries.
