Comments:
The Licensing Inspector for the Roanoke YMCA After School Program @ Crystal Springs Elementary School conducted an unannounced Renewal Inspection on Tuesday, December 17, 2019. Upon arrival at 3:25 PM there were a total of thirty-nine (39) children ages 5-10 years old present under the direct care & supervision of three (3) staff members. Five (5) children records were reviewed during today?s inspection. Two out of the five records reviewed were medication records. Staff records are maintained at the Roanoke YMCA home office and were reviewed on 12/18/2019. Licensing Inspector observed children during departure procedures, homework assistance, meals, bathroom/hand washing procedures, outdoor activities, group activities and self-initiated activities.

Violations found were reviewed during the exit interview with the Site Director on 12/17/2019 and were reviewed with the Program Director on 12/18/2019. Inspection conducted at 5:20 PM on 12/17/2019. Description: Based on medication record review, the Licensee failed to obtain parental written authorization to administer medication.

EVIDENCE:
Licensing Inspector reviewed two (2) children medication records.

1. Medication record for Child #1 did not include a current medication authorization consent form completed and signed by the parents or guardians. Medication authorization observed in the record for Child #1 expired January 2019.

2. Medication record for Child #2 did not include a medication authorization consent form completed and signed by the parents or guardians.

Plan of Correction: Parent will complete medication authorization form
