Comments:
A Subsidy Health and Safety inspection was conducted on December 17, 2019 to review supplemental health and safety requirements. Upon arrival at 3:35 PM there were a total of thirty-nine (39) children ages 5-10 years old present under the direct care & supervision of three (3) caregivers. Adult to child ratios were in-compliance upon arrival & throughout the inspection. LI Anderson observed children during arrival/departure procedures, bathroom/hand washing procedures, snack/meal served, group activities, outdoor activities, self-initiated activities and homework/quiet time.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations found during the supplemental subsidy health and safety inspection were reviewed with the Site Director on 12/17/2019. An exit interview was conducted with the Program Director on 12/18/2019. Inspection conducted on 12/17/2019 concluded at 5:20 PM. Description: Based on review of medication records, the Vendor failed to have a current written allergy care plan for each child with a diagnosed food allergy.

EVIDENCE:
Record for Child #1 did not include a current food allergy care action plan. Allergy care plan for Child #1 expired 1/2019.

Plan of Correction: Parents will update the food allergy care plan. Documentation will be maintained in the child record
