An unannounced monitoring inspection was conducted on Tuesday, August 27, 2019 to determine the center's compliance with licensing standards. The inspector arrived at the center at 9:50am and concluded the inspection at approximately 12:30pm. The center's director assisted the licensing inspector throughout the inspection. The census for today?s inspection consisted of 31 children in the direct care of 8 staff members. Upon the inspector?s arrival, the children were engaged in a variety of activities. The older children were outside playing kickball, riding bicycles, interacting with one another and the staff. The preschool children were also observed in their designated playground. Later, each group was observed during transitions, washing hands, preparing for lunch, and during a teacher-led activity. Staff were observed having positive interactions with the children. The playgrounds were inspected. The classrooms are decorated with the children?s artwork and are equipped with age and stage appropriate materials. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The first aid-kit and non-emergency supplies were inspected and found complete. Five children's records, one medication and authorization, and five staff records were reviewed. The most recently documented fire inspection was on 08/20/2019. The most recently documented health inspection was on 2/21/2019. The most recent practice evacuation drill was documented on 08/15/2019. The most recent practice shelter-in-place drill was documented on 06/05/2019.

Based on a review of five staff records and interview on 08/27/2019, the center did not ensure that one staff submitted tuberculosis (TB) test results at least every two years from the date of the first initial screening or testing.
Evidence: 1) The record for Staff #4, hired on 07/05/2016, contained TB screening results that were dated 04/27/2017. The screening expired on 04/27/2019. 2) During interview, a member of management confirmed that Staff #4 did not resubmit TB test results by 04/27/2019.
Plan of Correction:
Per the Center: The staff will be going to take the TB screening on 08/27/2019. In the future, management will continue to monitor the teacher requirement log to ensure compliance with deadlines.
