Thank you for your time and assistance with the unannounced monitoring inspection conducted today, November 18, 2019. The on-site inspection was from 10:15 a.m. to 12:30 p.m. The provider and one assistant were caring for ten children (28 points). There were a total of 12 children enrolled in the program. There were five children's records reviewed along with the records of the provider and four assistants. As is the policy of the provider, there were no medications currently being administered. The exception to compliance was noted on the violation notice and the risk rating assigned to the violation was noted on the supplemental page. Lunch was observed to meet USDA guidelines and consisted of fish sticks, peas, bread, pears, and milk. Some of the items reviewed and observed included: updated emergency preparedness plan, required posted information, documentation of evacuation, lock down, and shelter-in-place drills, injury reports, warm water temperature, refrigerator and freezer temperatures, diapering procedures, hand washing and bathroom procedures, transitions, and lunch.

Based on review of five children's records and verification from the provider, the physical examination report for one child was obtained after the first 30 days of enrollment.
Evidence: 1. The physical examination report for Child #4 was dated 03/06/2019. The first date of attendance for Child #4 was 01/14/2019. 2. The provider confirmed the physical examination report was obtained after the first 30 days of attendance.
Plan of Correction:
The provider will ensure that from now on the physical examination reports will be obtained within the first 30 days of attendance.
