Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a on-site tour of the program.
A renewal inspection was conducted on 3/29/2022. The director was contacted by email to initiate the inspection. There were 36 children present and 2 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 8 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on observation it was determined the center did not ensure that all areas and equipment of the center inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. There were 3 frayed carpets in the young toddler room. 2. The flooring in the bathroom of the 3-4 year old room was separating from the wall and was cracked. 3. There were some peeling paint in the 3-4 year old room on the bathroom door and on the outdoor play equipment. 4. There were frayed retractable gates in the center.
Plan of Correction:
Remove carpets and order new ones.
Director put a maintenance request to fix the chipped paint and remove gates.

Description:
Based on observation it was determined the center did not ensure that children under 10 years of age were always within sight and sound supervision. Evidence: Inspector observed the following during the observation of the 2-3 year old classroom: -One child hit his back on the underside of a table while the teacher was putting items away in the closet -One child hit another child in the back with a magnatile point while the teacher had their back turned attending to another child -One child swinging between two tables -Children fighting over materials In the above instances the teachers in the classroom were unable to maintain sight supervision on all of the children due to the set up of the classroom and therefore did not witness these events or did not attend to the instances quickly due to not being aware of the issues.
Plan of Correction:
Have several structured activities going at once split the class into 2 groups. Wait to put materials in the closet.
