Comments:
A Subsidy Health and Safety inspection was conducted on 7/9/19 from 2:30m - 4:15pm. During the inspection there were 23 children ages four to five years old in care with five staff. Children were observed participating in various activities in the gym, eating snack, and during rest period. Records were reviewed for five children and five staff during the inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-510-B
Description: Based on observation and interviews, it was determined that the vendor did not ensure that for each group of children, the vendor shall maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1. The Licensing Inspector reviewed the attendance records for the facility, and the attendance records did not have the departure documented for all children in care. 2. Staff #2 confirmed that there was not a written record of daily attendance that documented the arrival and departure of each child in care as it occurs.

Plan of Correction: The vendor responded: Staff will be re-trained on attendance procedures to ensure that arrival and departure of each child is documented on the attendance sheet. center management will double check the attendance record daily to make sure it is complete.

Standard #: 22VAC40-665-540-A
Description: Based on a review of five staff records, it was determined that the vendor did not ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Evidence: 1. The record for staff #4 (date of hire5/20/19) contain documentation of a TB screening that was dated 11/16/18. 2. Staff #2 confirmed that staff #4 had not completed a TB screening within the last 30 calendar days of the date of employment.

Plan of Correction: The facility responded: All newly hired staff will not begin employment until a TB screening has been completed, and it will have been completed within the last 30 calendar days of the date of employment.
