Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on March 11, 2021 and concluded on March 12, 2021. The director was contacted by telephone to initiate the inspection. There were seventy-two children present and eight staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed four children?s records and four staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.
Based on review, facility did not ensure 1 of 4 children's records have a physical examination and immunization records were signed by a physician, designee or an official of a local health department.
Evidence: The record for child #1 (start date 9/14/2020) had a physical and immunization record that was not signed by the physician, designee or health department official.

Based on review, facility did not ensure 1 of 4 staff have a national fingerprint Office of Background Investigations (OBI) letter of eligibility prior to beginning employment.
Evidence: The record for staff #2 (start date 9/23/2020) had a OBI national fingerprint letter of eligibility dated 12/4/2020 which is after start date of employment.
