Comments:
An unannounced renewal inspection was conducted on January 29, 2024 from approximately 1:20 pm to 2:55 pm. Six staff were present supervising thirty-three school age children. Children were observed engaged in various group led and individual choice activities. Five staff and five child records were reviewed. Violations found are noted on the violation notice.
Based on review, facility did not ensure 1 of 5 staff records has a completed central registry search within 30 days of beginning employment.
Evidence: The record for staff #1 (start date 10/9/2023) did not have a central registry search.
