Comments:
An unannounced Renewal Inspection was conducted today from 9:50am to 10:50am. There were 37 preschoolers directly supervised by 4 staff. The physical plant, staff records were current from the previous inspection visit, 5 children?s records, children's medications and medication authorization records were the same from the previous visit, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play,praying, preparing for snack, eating snack, washing hands. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: Observed two holes in the wall, large enough for a child's finger, in Pre-K 2.

Based on observation, the facility did not ensure that electrical outlets were covered at all times. Evidence: Observed 1 uncovered electrical outlet in Pre-K 1, near the hallway door, on the other side of the children's shelf.
