Comments:
An unannounced monitoring inspection was conducted today from TIME. There were 38 (3-5yrs) directly supervised by 4 staff. The physical plant, 5 children?s records, 1 child's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, eating snack and playing outside. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Based on observation, the facility did not ensure that when preschoolers are in care, electrical outlets were covered with protective covers. Evidence: observed in Pre-K 2, an uncovered electrical outlet near the sink.

Based on review, the facility did not have documentation of medical authorization for medication that was kept on site. Evidence: reviewed 1 child's medication authorization record and found that there was no physician or parental medication authorization for the child's medication that was kept in the nurse's office.
