Comments:
An unannounced monitoring inspection was conducted today from 8:57am-9:36am with the provider on site. There were 9 children with 3 caregivers. A sample of 2 children's files and 4 caregivers/household member's files were reviewed. The children were engaged in the following: playing outside. Interactions between the children and caregivers were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. The physical space, attendance records, first aid kit, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Based on review, the provider did not obtain the parent's written acknowledgement of the receipt of the information required regarding the Provisions of the Emergency plan. Evidence: 1. Child #2 (start date 2/13/2023) did not have the parent's signature on the document labeled Provisions of the Emergency Plan.

Today I will give to the parent and have them sign the form.

Description:
Based on observation, furnishings of the family day home outside were not maintained in a safe and operable condition. Evidence: 1. A children's play slide structure had a large crack on the base of the structure at the top.
Plan of Correction:
I will remove the slide and get rid of it Monday.
